The format of Haj application form will also be available on the website, which can be downloaded and applied offline as well. Photocopy of the format will also be valid.

Printout of filled forms will be sent to the State Haj Committee, 10A, Legislative Assembly, Lucknow by hand or by post for verification including attached forms. On the address of State Haj Committee, including offline forms attached forms, it is necessary to get it by 7th December.

Singh said that in the normal category, the address given in the form lodged in the form and address in the passport is one, then the self signed photo copy of the passport must be attached. Singh said that the copy of the bank account’s cancellation check with the application form, photocopy of the bank passbook, one color photo (white background), declaration on the prescribed format, pay-in-slip of the deposit fee (original copy), passport Photocopies (all photocopies are self-signed) must be submitted.
